In order to help you keep track of those important volunteer hours, we have developed a Fraternal Activity Reporting Form. Just use one of these forms for your committee meetings, camp/lodge meetings, fraternal events and community service events. For those events that involve more than one planning meeting, here is a Fraternal Activity Summary Reporting Form to compile information from the individual Fraternal Activity Reporting Forms.
