Chronic Illness in Seniors

Resources for chronic conditions suffered by seniors.  Chronic conditions are impairments or illnesses that have no cure but result in persistent or recurring health consequences.

 American Association of Diabetes Educators
AADE  is a multidisciplinary association of healthcare professionals dedicated to integrating self-management as a key outcome in the care of people with diabetes and related chronic conditions.

American Cancer Society (ACS)
(ACS) is committed to fighting cancer through balanced programs of research, education, patient service, advocacy, and rehabilitation.

American Diabetes Association (ADA)
ADA's mission is to prevent and cure diabetes and to improve the lives of all people affected by diabetes.

American Geriatrics Society (AGS)
The American Geriatrics Society (AGS) is a not-for-profit organization of over 6,400 health professionals devoted to improving the health, independence and quality of life of all older people.

American Heart Association (AHA)
AHA's mission is to build healthier lives, free of cardiovascular diseases and stroke.

American Public Health Association (APHA)
The American Public Health Association is the oldest and most diverse organization of public health professionals in the world and has been working to improve public health since 1872. The Association aims to protect all Americans, their families and their communities from preventable, serious health threats and strives to assure community-based health promotion and disease prevention activities and preventive health services are universally accessible in the United States.

Arthritis Foundation (AF)
The Arthritis Foundation is the only national not-for-profit organization that supports the more than 100 types of arthritis and related conditions.  Founded in 1948, with headquarters in Atlanta, the Arthritis Foundation has multiple service points located throughout the country. The Arthritis Foundation is the largest private, not-for-profit contributor to arthritis research in the world, funding more than $380 million in research grants since 1948. The foundation helps people take control of arthritis by providing public health education; pursuing public policy and legislation; and conducting evidence-based programs to improve the quality of life for those living with arthritis.
